Roadway design and improvements

I have recently been asked to investigate a single vehicle accident in which roadway signage (or lack thereof), and a recently installed guard rail have become an issue.

Ther is a right curve sign posted and visible for a right curve in the roadway, then a straight section of road (500 feet), then a decreasing radii right curve with no signage. In brief, the vehicle entered the decreasing radii right curve on wet asphalt losing control and running off the left side of the roadway (no guard rail). The driver was unfamiliar with the roadway. Since the accident a guard rail has been installed and reduced the amount of serious accidents, according to residents.

My questions are;
What determines when guard rails are installed?
Why isn't a sign posted for the second curve (or at least an "S" sign at the first curve)?
Where can I obtain such information?
