This happened to me, but not with an insurance company. the client was a private individual who refused to pay because he didn't agree with my analysis.
I took him to small claims court. The judge saw my work product, and I offerred to play audio tapes of the client's voice mail messages. The judge had already decided in my favor after seeing the work product though, so I won the case easily. Our company policy is now to ask for a $500 retainer fee when hired by a private individual.
