Crash Form Computerization

The department for which I work is currently evaluating the feasibility of converting the state crash reporting form for use on computers. I am interested in finding software that will scan the current form and allow for the spaces to be set up as fields for data entry. The ideal program would be simple and allow for pull-down menus for frequently used data in fields such as "primary cause" and "wrecker service".

I know that several programs of this type exist, but am interested in feedback and suggestions regarding specific software.
